The IgE monoclonal gammopathies include malignant plasma cell disorders such as IgE multiple myeloma (MM), as well as the associated premalignant condition IgE monoclonal gammopathy of undetermined significance (MGUS). We report a case of a 41-year-old woman presenting with an IgE kappa monoclonal protein following routine laboratory testing. Serum protein electrophoresis (SPEP) initially showed a monoclonal protein in the beta-2 fraction, at an estimated concentration of less then 4 g/L. Subsequent serum immunofixation electrophoresis (SIFE) including antisera to Ig heavy chains delta and epsilon confirmed the presence of an IgE kappa monoclonal protein. Analysis of serum free light chains (FLCs) showed increased levels of kappa FLC, resulting in an abnormally elevated kappa/lambda FLC ratio. No Bence-Jones proteinuria was present. Bone marrow aspiration showed 6% plasma cells, and no sign of myeloma-associated end-organ damage was evident. Consequently, the patient was diagnosed with IgE kappa MGUS. In the present report, the clinical characteristics of the patient are compared to previous descriptions of IgE monoclonal gammopathy. The report further emphasizes the importance of considering the presence of monoclonal IgD or IgE when SIFE shows a clear band positive for a light chain but is negative for Ig heavy chains gamma, alpha and mu. Attention deficit/hyperactivity disorder (ADHD) is associated with impairment in social, academic and job-related functioning in both children and adults. The purpose of this study is to investigate the susceptibility to cardiovascular risk factors in children with ADHD diagnosis through common carotid artery intima-media thickness (cIMT) measurement, to compare cIMT in ADHD and control groups and to evaluate the association between cIMT and ADHD symptom severity. The mean cIMT of 42 children with ADHD, ADHD symptom scales and a semi-structured psychiatric interview, and 42 age and sex matched healthy controls were measured with B-mode Doppler neck ultrasonography. The median cIMT was significantly higher for the ADHD group compared to the healthy controls. There was a statistically significant, negative, moderate correlation between cIMT and Conners ADHD index score, hyperactivity score, oppositional score and the presence of ODD comorbidity. AGA can be implemented with the support of a health professional to provide feedback to older adults on physical and cognitive function.Objective To evaluate the effects of AGA.Methods For this feasibility study, we enrolled 23 and 20 participants in the intervention and control groups, respectively. Thοse in the intervention group received feedback of their assessments and devised exercise plans with professional support. The participants performed their exercises over 12 weeks; the follow-up examination was conducted at 40 weeks. The control group only received feedback. The change in health-related consciousness and behavior was evaluated. Muscle strength and mass were measured and the timed up and go test, and the Brief-balance evaluation system test (BESTest) were performed.Results There were no significant differences in consciousness or behavior between the groups. The score was higher in the intervention than in the control group at 40 weeks in Section-I of the Brief-BESTest, indicating that the muscle strength contributed to balance function.Conclusion AGA did not show positive changes in consciousness or behaviors but demonstrated significant improvements and lasting effects in balance function.We aimed to assess the impact of the Covid-19 pandemic on the risk perception and attitudes concerning influenza and pneumococcal vaccination in an at-risk population. We performed an online survey between July 29th and October 12th 2020 in patients at risk of severe influenza and/or pneumococcal disease recruited from Ipsos internal panel or through AVNIR (Associations VacciNation Immunodéprimées Réalité), a group of patient associations. Among the 2177 respondents, 1350 were female (62%), median age was 58 years (IQR 47-69), 62% were immunocompromised. In total, 86% of respondents declared being favorable to vaccination in general and felt at high-risk of more severe forms of Covid-19 (81%), pneumococcal disease (76%) and influenza (74%). More than half (52%, 388/746) of patients were eager to be vaccinated annually against influenza for the next 2020/2021 season and pneumococcal disease in the future 62% (376/606). In this sample of patients at-risk of severe forms of influenza, pneumococcal diseases and Covid-19, the self-perception of risk for these three diseases and positive opinion on vaccination in general were high.
